Collaborative Treatment and Recovery Team

Eligibility Requirement: Sonoma County Resident. You must have a referral and a diagnosis from Sonoma County Behavioral Health

Collaborative Treatment and Recovery Team (CTRT) is a program that helps first-time behavioral health services clients gain and maintain their independence in the community, individually or in shared households.

  • Provides system navigation and case management services to assist clients in securing and maintaining support services to improve their quality of life and level of self-sufficiency.
