ISHP supports referred Sonoma County adults with severe and persistent mental health challenges as they transition from long-term 24-hour care to more independent living. While residing in a group setting, clients receive intensive case management and wraparound services to build stability and essential life skills.

Sonoma County residents aged 18+ with referral and diagnosis from Sonoma County Behavioral Health
FACT supports adults with severe and persistent mental health needs referred to us by the criminal justice system through the Sonoma County Behavioral Health FACT program. We offer non-violent individuals the opportunity to live independently in the community with specialized care, including case management, independent living skills training, and program management.
