Forensic Assertive Community Treatment (FACT)

Eligibility Requirement: Sonoma County Resident. You must have a referral from probation or court order along with a diagnosis from Sonoma County Behavioral Health

Forensic Assertive Community Treatment (FACT) provides intensive case management services to clients who are involved in the criminal justice system and receiving services from Sonoma County’s FACT program. The program offers individuals the opportunity to live independently in the community with support from Buckelew staff.
